Background:Bacterial infection is common and has been associated with a range of adverse effects. We evaluated the use of azithromycin, a commonly used antibiotic with a high incidence of side effects, in a large randomized clinical trial. This is an open-label study of 16 patients with bacterial pneumonia, who were randomly assigned to receive either 2 grams of azithromycin or a placebo for one week. Patients were evaluated at baseline, after three weeks of treatment and at 24 weeks. The primary endpoint was the incidence of clinical cure in the azithromycin group and in the placebo group, as well as in the treatment group. A secondary endpoint was defined as the difference in the rate of clinical cure (as measured by the number of days at least 3 to 9 at week 0 and weeks 2 and 3 to 5 at week 24) between the two groups. The results of this study were statistically significant for the main primary endpoint. The results also showed a reduction in the number of days at least 3 at week 24 in the azithromycin group, although not significant. The study did not find a statistically significant difference between azithromycin and placebo in the rates of adverse events related to the treatment group. The results of this study will be of value in guiding future clinical trials of this antibiotic. We hope that this study will be of value in guiding clinical trials for the use of azithromycin.
